The AFC Cup began life in 2004 to compliment the AFC Champions League and offer clubs from countries with developing status a taste of continental competition. 18 teams from 14 countries competed in the inaugural tournament with Al-Jaish taking the title after they defeated fellow Syrian club Al-Wahda on away goals.

Developing nations in Asia are categorised as the fourteen 'next-best' countries after the top-14 nations in the AFC region, with the latter's club's gracing the prestigious AFC Champions League. The finalists of the AFC Cup compete in the qualifying round of the AFC Champions League provided they meet certain criteria.

Kuwaiti club Al-Kuwait are the most successful club in the competition's history with three titles to their name. Kuwait is also the most successful country in the competition with four titles. Clubs from West Asia have also dominated the tournament to date.

The competition has expanded to 36 teams for the group stage with 12 from West Asia and ASEAN, and 4 from East Asia, Central Asia and South Asia.
